Pediatric and NICU Case Management & Discharge Planning
Case management is the process of coordinating resources that will help you continue to care for your child in your home based on their individual needs. The discharge planning process actually begins at the time of your childs admission to Sunrise Childrens Hospital. Members of your childs healthcare team, including the physicians and nurses will work closely with you and your child to prepare for any discharge needs, including arranging medically necessary home nursing care and home medical equipment.
Here are some suggestions to help you prepare for this day:
- Let your nurse or doctor know of any problems you identify associated with going home.
- Make sure your childs nurse has all of your contact numbers so that we may notify you of your childs discharge or to discuss any special home care needs identified by a member of your childs healthcare team.
- Be available to take your child home on discharge day. If you need a work excuse letter, please let your childs nurse know.
- Anticipate any special transportation needs and make those arrangements
- Be prepared to give your nurse or physician the name, number and address of the pharmacy your family uses, as well as any preferences you have for home care services.

SCAN Team
The SCAN (Suspected Child Abuse and Neglect) Team at Sunrise Childrens Hospital is a multi-disciplinary group of professionals specializing in identifying and evaluating suspected victims of child abuse. The team includes pediatric emergency department physicians, a child abuse physician specialist, nurses, pediatric social workers, child life specialists and members of the community from law enforcement, child protective services and the district attorneys offices. Investigators utilize space in the department to interview children and speak to caregivers.
Our health care staff members collect potential evidence during a comprehensive medical examination performed by a physician or advanced practice nurse. If you suspect your child is a victim of child physical or sexual abuse, report it to the Clark County Child Abuse and Neglect Hotline at 399-0081 or contact the Pediatric Emergency Department at 702-731-8181 for further assistance.
